If they see it coming, wouldn't they have the opportunity to declare a normal Defends action at their full dice pool, even without Reflexive?

Close. According to Reflexive, if you have the Defends quality and you are attacked by surprise, you get to make a free defensive roll at -1d, as long as the attack isn't Talent powered.

But if an ubermensch throws a grenade at you with his Hypercoordination, you're back to defending and attacking as multiple actions.

(It's probably a bit too "gamey" to suggest that if you see it coming you can't use Reflexive as a free action for a -1d penalty. I'd allow it, but not against Talent attacks.)

If you don't want the -1d penalty, you would have to declare "Catch" as an action. If you want to "Catch" an attack by a Talent, you would have to declare "Catch" as an action. For things like grenade attacks from normal attackers, 3hd (4hd with a -1d penalty) is probably good enough, so you could let Reflexive defend you with a free action and attack with your normal action.

That would be pretty cool, walking through explosions and firing them back at the enemy.
